The key is that he stay positive and that he doesn't alter his motion or his mechanics to compensate for his lack of control or his ankle. If he does that, he risks injury to his arm and I would rather see him shut down than have that happen.
I'm sure the Sox will monitor this closely. I think the end of August may be a bit ambitious, but as I learn about Peavy, I know he is a competitor of the highest order. Again, don't let the desire to be competitive get in the way of making sure that he's healthy before they trot him out on a major league mound.
